Use this form to report a safeguarding concern or incident to the MMJFC Club Welfare Officer. All reports are confidential and can be submitted anonymously.

📋 What happens when you report?
- Your report goes directly and confidentially to the Club Welfare Officer
- You do not need to provide your name or contact details
- The CWO will assess the concern and act in line with the club's Safeguarding Policy
- For serious concerns, the CWO will liaise with the County FA Designated Safeguarding Officer
