Injury Report
For coaches and managers to log player injuries sustained during training or match days. All reports are kept on record for the 2025/26 season.
This form is for coaches and managers only.
📋 What to include
- Your name and the team the injury relates to
- The player's name and date of birth
- Date of the injury and a description of what happened
- First aid treatment administered on the day
- Any advice or recommendations given to parents or carers
🚨 In an Emergency
This form is for logging injuries after the event. If a player requires immediate medical attention:
- Call 999 for an Ambulance
- Administer first aid while waiting for emergency services
- Contact the player's parent or carer immediately
- Notify the club secretary as soon as possible
